Requirements: Barangay clearance, business name certificate, picture of business site, picture of signage, registration fee
4. Tax Identification Number - BIR (Bureau of Internal Revenue)
Requirements: Accomplished Form 1901, Mayor's Permit, payment of registration fee
5. SSS Certificate (accomplish the R1 form, submit copy of Business Name Certificate from DTI, xerox copy of Mayor's Permit)
For Partnership:
1. SEC (Securities and Exchange Commission) certificate
Requirements: Verification slip, registration data sheet, articles of partnership, written undertaking to change corporate name when necessary, payment of registration form
